HEALTH IMPROVEMENT PLAN NURSE
Improves the health of Company associates. Administers HIP at facility to include administration of initial Health Risk Assessment (HRA), vital signs, measurements, lab draws, health education & counseling, program assessment & evaluation.
PRINCIPAL AND ESSENTIAL DUTIES AND RESPONSIBILITIES
- Performs interviews for HRA and assists with completion of form. Measures vital signs, weight and height and record. Draws blood for appropriate/standard lab work (if competent), complete lab forms, prepare samples and send to the lab.
2.Manages the implementation of all aspects of the health promotion program. Reviews results of HRA with associate, identifies areas of risk and establish a Personal Plan for Health (PPFH) for the individual. Utilizing the principles of QIP, performs periodic
follow up of PPFH with associate.
3.Uses and analyzes data to assess and target health promotion needs for the employee population. Participates in marketing programs to increase associate awareness.
4.Provides support to Wellness Center/Occupational Health Department as directed by Wellness Center supervisors to insure overall function of department.
5.Assesses needs of the population, plans and implements health education to meet these needs. Serves as a resource for health and wellness related information for associates.
PREFERRED: Bilingual (English/Spanish) is a plus.
REQUIRED: Bachelor’s degree in health education, health promotion, etc. plus 5 -6 years related work experience or graduate of accredited program of professional nursing, practical nursing or combination of education and employmentrelated to health promotion, e.g., nutrition, exercise physiology. If an RN or LPN, must have a current license in the state of practice. For non-nursing applicants, CHES or health coaching certification; occupational health and/or health education. Clinicalskills to include data collection, venipuncture, vital sign monitoring. Requires excellent verbal and written communication skills and a belief in improved health through reduction of controllable risk factors.
Position is active; may require a good bit of walking around, moving to other offices or buildings and through the plant.
May need to move light equipment or supplies from one place to another.
May need to access files, supplies and equipment.
Work activity is generally in a office, clinical environment.
When in a plant environment:
Exposure primarily consists of wet and moist floors which include metal and plastic grating surfaces.
May be exposed to temperatures of 28 degrees to 100 degrees Fahrenheit with both ambient and 100% humidity.
May handle product 25 degrees to 50 degrees Fahrenheit.
May be exposed to noise ranges of 50 db to 110 db.
May be exposed to all chemicals found in poultry, food and processing plant.
Must wear and use protective and safety equipment required for the job as directed by the Company.
